1017-1(a)(3): Conversion: CHAPTER 11 to 7--12--13: Motion Required
LBR 1017-1(a)(3) provides the procedure for a chapter 11 debtor to request an order converting a chapter 11 case to another chapter.

Before a chapter 11 case is converted, all pending court costs and U.S. trustee fees must be paid. When a motion to convert a chapter 11 case is filed, the court may file a Notice of Court Costs if court costs remain unpaid and the U.S. trustee may file a notice to indicate the amount of U.S. trustee fees that remain unpaid.
FRBP 1019 provides instructions on the documents that must be filed after a chapter 11 case is converted or reconverted to a chapter 7 case.
