3015-1(b)(5):  Chapter 13:  Claims:  PROOF OF CLAIM

LBR 3015-1(b)(5) requires that when creditors file a proof of claim, they must prepare and file the mandatory proof of claim form.

The Proof of Claim can be filed in three ways -- see links below:

  • mail a copy to the clerk's office
  • file a scanned version via CM/ECF
  • submit it using the electronic proof of claim process (ePOC)
