2090-1(a)(3)-(4):  Attorneys: Partial Representation: Disclosure of Representation

LBR 2090-1(a)(3) requires attorneys to file a form that discloses the attorney's compensation arrangement with an individual who files a chapter 7 bankruptcy case.

• This form is mandatory and must be filed immediately at the commencement of the chapter 7 bankruptcy case.  
• The form requires the attorney to disclose the compensation arrangement, the source of compensation paid postpetition, and any sharing of compensation paid postpetition.
