9009-1: FORMS: Three Categories: Used After a Bankruptcy Case is Filed
LBR 9009-1 provides information about three important categories of forms that are used after a debtor opens a bankruptcy case. The Three Categories of Forms are:
1. Forms that Apply to All Bankruptcy Cases Nationwide -- LBR 9009-1(a): See link below to "Official Forms", which are published by the Administrative Office of U.S. Courts.
- Official Forms are published by the Administrative Office of U.S. Courts for use by all bankruptcy courts in the United States.
- Most Official Forms have a naming system that begins with the words "Official Form" or the letter "B", for bankruptcy. For example, Official Form 101 or form "B 101" is a voluntary petition to individuals filing a bankruptcy case.
2. Forms that Match a Specific Local Bankruptcy Rules (LBR) -- LBR 9009-1(b): See link below to LBR Forms, a complete list of forms that match Local Bankruptcy Rules.
- All LBR forms begin with the letter "F" in the bottom right side of the form. An example is F 9013-1.1.HEARING.NOTICE and its related LBR is LBR 9013-1(d).
3. Forms that Do Not Match a Specific LBR -- LBR 9009-1(b): See link below to "Other Forms" that are very useful, but do not match a LBR.
- Examples are subpoena, abstract of judgment, and reaffirmation agreement.
